The COVID-19 pandemic has posed an unprecedented challenge for healthcare systems around the world. The second wave was particularly alarming given how it multiplied the demand for a host of medical equipment, especially oxygen concentrators; it became necessary for people to keep them in their homes due to short supply. With the third wave of the pandemic looming large, it only makes sense to understand this piece of equipment and its uses.
Dr Veena Aggarwal, Gynaecologist, Consultant Women’s Health, Medtalks.in, MD IJCP Group, and Trustee, Heart Care Foundation of India, says, “An oxygen concentrator helps oxygenate patients when their blood oxygen levels (SpO2) drop below 94 per cent. It helps provide a continuous supply of oxygen to those with respiratory issues and does not require replacement or refilling, concentrating oxygen from ambient air.” She lists a few things to keep in mind while using oxygen concentrators.
There are two types of oxygen concentrators: continuous flow and pulse dose. The right device should be used based on suggestions by the specialist. For patients who are under home care, oxygen concentrators with 3-5 litre per minute (LPM) capacity are ideal.
The concentrator must be placed one foot away from the bed without any obstructions around it. The humidification bottle must be filled with distilled or filtered water, and the threaded cap of the bottle must be fitted inside the outlet of the concentrator. The oxygen tubing should be attached and tightened to either the humidification bottle or oxygen adapter. It is important to turn on the concentrator at least 15-20 minutes before use since it requires time to cycle the correct concentration of air.
Set the litre control knob as per the prescribed LPM; in case you are not sure how to do this, consult a specialist and avoid adjusting the LPM randomly. Make sure that the tube has no bend or kink, and that there is no gap in the sides of the mask that is used for breathing.
When the oxygen concentrator is on, it makes a loud sound, which indicates that the air is being processed. Make sure to check the light indicator which is an indication of whether the machine is working properly or not. Avoid using an extension cord for plugging the concentrator because it requires a lot of power for operation and could be risky under circumstances otherwise.
The mask used must fit the person’s face properly without any gaps around that might lead to leakage. It should be of a proper size and the nose clip and tightening straps must fit snugly for uninterrupted oxygen supply. The nasal cannula must be adjusted upward in the nostril for better oxygen levels.
During COVID-19, medical resources may be limited in many settings. This means that one may need to borrow or rent the cannulas and concentrator machines for use. Both caregivers and patients’ families must ensure that the equipment is well-sanitised before use.
Apart from remembering these pointers, Dr Aggarwal advises, “One should keep track of their oxygen levels and watch out for any signs that indicate complications. In case of any adverse symptoms, make sure to consult a specialist.”
